This is the kind of question where I think people are putting the cart before the horse. With all the influx of new fillers and distribution to anyone who is willing to use it, the artistry is being lost in aesthetic medicine. At the end of your consultation, the plan should be to bring you to a more beautiful, pleasing appearance, not just inject fillers. For almost everyone, a combination approach makes more sense and provides a more harmonious appearance.
So, what is the goal? To simplify, it is important to have smooth, even skin that is lively and bright, coupled with loss of redundancy, and shaping of the face into a nice square jaw for men and heart shaped, soft curve for women. This is an over simplification, but the end goal needs to be kept in mind or no filler or facelift will give you the proper result.
Fillers play an excellent role to achieve these aesthetic goals. Softening the nasolabial fold, improving jaw definition, softening the temporal hollow, filling a hollowed upper lid, and building up the lid cheek junction. Depending on the goal, various fillers are better.
For simulating a bony augmentation, such as the lateral jaw, my favorite filler is Sculptra. It creates your own collagen which tends to be a bit firmer and better emulates the feel of bone. This works great in the midface. Voluma is an excellent filler for this region also.
For the nasolabial folds hyaluronic acids are one of the best. I prefer a thicker hyaluronic acid such as Restylane Lyft or Juvederm Ultra. This area is relatively forgiving and the thicker gels give a longer lasting result.
For trickier areas such as under the eyes, softer, more liquid fillers such as Restylane Silk is an excellent choice. With the think skin of the eyes, these softer fillers are more forgiving. This particular filler is also excellent for fine lines and wrinkles around the lips and crow’s feet.
For lip augmentations I prefer middle weight fillers such as Restylane or Juvederm. These provide longer lasting results, but the softness helps balance longevity with natural results.
